2> Fill up the form

Fill up all the mandatory fields

* For uploading documents, you need to have all documents: Photos, ID proof, Address proof in image format (i.e *.jpg, *.jpeg) as shown in below image.

*After filling up all the details & uploading documents

Click on "save" first, a documents will be downloaded.

3> After clicking "submit'", a new webpage / screen will open, which gives a "new web link" which states to know how to track your request for the requested change , as shown below:

​After clicking on the link, new webpage will open, as shown below:
